Dear FDF Directors,
The 2012 Folk Dance & Choral Festival is just around the corner and are
looking forward to having you be a part of another great year! The
following is information on how to submit your FDF 2012 Group FORMS.
Please make sure you have read the Director’s Manual for 2012 since there are
significant changes to bringing props to the event. (please see Director’s
Manual tab under the Director’s Arena section)
All Directors bringing groups in Dance Competition for Division I, Division II,
Division III, Division IV and all groups in Choral Competition in Division I
and Division II are required to submit the following forms by the noted deadline
dates to be eligible to participate at FDF 2012.
1) ROSTER SUBMISSION VIA EMAIL
When you have completed your group roster form, save it to your computer
with the following name format in ALL CAPS: 
DANCE GROUPS: "GROUP NAME_ROSTER" (i.e.-
NEA ELLAS_ROSTER)
CHORAL GROUPS: "GROUP NAME_ROSTER_CHORAL " (i.e.-
HARA_ ROSTER_CHORAL)
DIVISION III GROUPS: "GROUP NAME_ROSTER_DIVIII" (i.e.-
SPITHES_ROSTER_DIVIII)
DIVISION IV GROUPS: "GROUP NAME_ROSTER_DIVIV" (i.e.-
AXIOS_ROSTER_DIVIV)
Group Roster Form is your team registration for Choral
and Dance Groups, due January 7,
2012.

NOTE: You will be PENALIZED 1% of your total score if
your forms are submitted past the deadline.
2) ROSTER SUBMISSION VIA STANDARD MAILEach group is also required to print out a hard copy of your FDF
2012 Group Roster Form, have it signed by your parish priest
and sent via standard mail to the following address--postmarked
by January 25, 2012

Please do not write anything else in the address simply put the exact address
as it is listed above.
NOTE: You will be PENALIZED 1% of your total score if
your roster is postmarked past the deadline.
3) PERFORMANCE AND PROPS FORM (same form, two
separate tabs at the bottom of form)
When you have completed your performance form and if you
are bringing props please fill out the props form in the 2nd tab of the
document attached below, save it to your computer with the following
name format in ALL CAPS:
DANCE GROUPS: "GROUP NAME_PERFORM" (i.e.- NEA
ELLAS_PERFORM)
CHORAL GROUPS: "GROUP NAME_ PERFORM_CHORAL " (i.e.-
HARA_PERFORM_CHORAL)
DIVISION III GROUPS: "GROUP NAME_ PERFORM_DIVIII" (i.e.-
SPITHES_PERFORM_DIVIII)
DIVISION IV GROUPS: "GROUP NAME_PERFORM_DIVIV" (i.e.-

4) SUBMISSION OF COSTUME FORMThe FDF 2012 Costume Forms must
be submitted by every groups participating group at FDF 2012 and turned in
onsite at the Director’s Briefing on Thursday, February 16, 2012 along with
printed out photos of each costume. Only a front and back photo of each costume
will need to be submitted on an 8.5” x 11” piece of paper.

EMAILING INTO THE competition@yourfdf.org EMAIL
ADDRESS:
When emailing to the competition@yourfdf.org email address, please title
your email appropriately. For example, when emailing in your Performance
form for your Choral or Dance Group, title your email as "AXIOS,
Division III Performance Form" or "HARA, Choral Performance
Form" or "ZOI, Dance Performance Form".
**If you have sent your form in already and need to make a change, simply
update your form and resend it in and note in your email that there is
a change. No new/updated forms will be accepted after February 10, 2012,
all changes will need to made onsite.
