FDF 2010 - Performance and Costume Forms

Please review and email the appropriate forms to the following: competition@yourfdf.org:

• Costume Form
• Choral Performance Form
• Dance Performance Form

Dance Groups - Performance Form, Costume Form and only two photos per costume per performance.
Choral Groups - Performance Form
DIVISION III Groups Directors: Performance Form, but no Costume Form is needed.

Performance and Costume Forms are DUE emailed to this email address by December 28, 2009 or you risk being penalized 1% of your total score.

PHOTOS:

• You must submit only two photos per costume per performance day.
• One photo of the front and one photo of the back of the costume.
• Please clearly explain what costume photo is for what performance day.
• All groups must submit photos of their costumes.

NAMING YOUR SUBMITTED PERFORMANCE FORM DOCUMENT: 

When you have completed your group roster form, save it to your computer with the following name format in ALL CAPS: "GROUP NAME_PERFORM" (Example: NEA ELLAS_PERFORM)

NAMING YOUR SUBMITTED COSTUME FORM DOCUMENT:

When you have completed your group roster form, save it to your computer with the following name format per each day you'll be performing in ALL CAPS: "GROUP NAME_COSTUME_SEMI or FINAL" (Example: NEA ELLAS_COSTUME_SEMI or NEA_ELLAS_COSTUME_FINAL)

EMAILING INTO THE competition@yourfdf.org EMAIL ADDRESS:

When emailing to the competition@yourfdf.org email address, please title your email appropriately. For example, if you are sending in your Performance and Costume form for your Dance Group, title your email as "AXIOS, Division III Group Performance Form" or "HARA, Choral Group Performance Form" or "ZOI, Dance Performance, Costume Forms and Photos". And if you have a simple question please simply title your email.

SIGNING YOUR EMAILS SENT TO competition@yourfdf.org EMAIL ADDRESS:
When you send an email to competition@yourfdf.org, please sign your email with your name, group name, Parish and city. This helps us understand who the email is coming from, who to follow up with and organize the 100's of emails and paperwork we receive. And also helps confusion with groups with the same name but from different parishes.

**Please do not send more than one groups Performance Form and Costume Form in your one email. Please use separate emails for different groups.

**If you have sent your form in already and need to make a change, simply update your form and resend it in and note in your email that there is a change.
